If I were asked to name the single most important active ingredient in a skincare regimen — beyond SPF — it would be vitamin C.

Not because it's fashionable, and not because every brand sells a version of it. But because the clinical evidence for what it does to the skin at a cellular level is genuinely strong, and because it directly supports the same biological processes that in-clinic treatments are designed to stimulate.

That said, not all vitamin C products are the same. The concentration matters. The form matters. And understanding why is the difference between a product that genuinely changes your skin and one that does very little at all.

What vitamin C actually does in the skin

Vitamin C — in its active form, L-ascorbic acid — plays two distinct roles in the skin, both of which become increasingly important as we age.

The first is collagen synthesis. Vitamin C is an essential cofactor for the enzymes responsible for stabilising the collagen molecule. Without adequate vitamin C, collagen cannot be properly formed or cross-linked — it becomes structurally weak and breaks down more readily. Topical vitamin C at effective concentrations has been shown in clinical studies to upregulate collagen production in the dermis, improving skin firmness, reducing the appearance of fine lines, and supporting the skin's structural integrity over time.

The second is antioxidant protection. The skin is continuously exposed to free radicals generated by UV radiation, pollution, and other environmental stressors. These free radicals damage collagen, disrupt the skin barrier, and accelerate visible ageing. Vitamin C is one of the most potent topical antioxidants available, neutralising free radical damage before it can take hold. This is why it is best applied in the morning, before sun exposure — it works alongside your SPF to give the skin a layer of chemical defence that SPF alone cannot provide.

There is a third benefit worth mentioning, though it is often the one patients notice most quickly: vitamin C inhibits the enzyme tyrosinase, which is responsible for melanin production. This means it gradually brightens the skin, reduces the appearance of pigmentation and sun damage, and evens out skin tone — often within four to eight weeks of consistent use.

Why the form matters

This is where many patients are quietly let down by their skincare. Walk into any pharmacy or department store and you will find dozens of serums marketed as vitamin C. Many of them contain vitamin C derivatives — ascorbyl glucoside, sodium ascorbyl phosphate, ascorbyl tetraisopalmitate — rather than L-ascorbic acid itself.

These derivatives are more stable and less likely to cause irritation, but they have to be converted into active L-ascorbic acid by enzymes in the skin before they can work. That conversion is inefficient, variable, and often incomplete. The result is a product that is gentler to use but significantly less effective at driving the collagen-stimulating changes that the research supports.

L-ascorbic acid is the gold standard. It is unstable — it oxidises when exposed to light and air, which is why quality vitamin C serums come in opaque or airless packaging and should be stored carefully. When it oxidises, it turns yellow or orange, loses its efficacy, and can potentially cause irritation. But when formulated correctly and used consistently, it is substantially more effective than any derivative.

It also needs to be formulated at a low pH — below 3.5 — to penetrate the skin properly. This is what gives some vitamin C serums a slight tingle on application. It is not irritation; it is the acidity required for absorption. If your vitamin C serum feels completely neutral on the skin, it is worth questioning whether it is actually reaching the layers where it needs to work.

Concentration: why strength matters

This is the question I am asked most often about vitamin C, and the answer is nuanced — more is not always better, but below a certain threshold, the clinical benefits simply do not materialise.

The optimal range supported by clinical research. Maximises collagen synthesis and antioxidant protection without necessarily increasing irritation, provided the skin has been introduced gradually.
Above 20%
Absorption does not increase proportionally above 20%, and the risk of irritation rises. Higher concentrations are not demonstrably more effective and are not routinely necessary.

In practice, I recommend most of my patients begin at 10% and build up to 15–20% over time, once the skin has adapted. Jumping straight to a high concentration on unprepared skin can cause redness, stinging, or transient irritation — which leads people to abandon a product that would have worked well with a more gradual introduction.

The connection to injectable treatments

This is something I feel strongly about, and it is a message I give to every patient I see in clinic.

In-clinic treatments — whether that is Sculptra stimulating new collagen, Profhilo improving tissue quality, microneedling triggering a repair response, or skin boosters supporting dermal hydration — all work by stimulating the skin's own biological processes. They create a window of opportunity in which the skin is actively regenerating and rebuilding.

What you do at home during that window, and in the months that follow, determines how well those results are maintained.

A patient who has Sculptra to restore facial volume and then goes home to a skincare regimen that does nothing to support collagen is, in effect, working against the treatment they have just invested in. The collagen stimulated by the treatment will still degrade over time — that is an unavoidable biological process — but the rate of degradation is influenced by sun exposure, free radical damage, and the availability of the nutritional cofactors the skin needs to produce new collagen.

Topical vitamin C addresses all three of those factors directly. It provides antioxidant protection against free radical damage, it directly supports collagen synthesis, and when combined with a broad-spectrum SPF, it significantly reduces the UV-driven collagen breakdown that is the single largest external contributor to visible skin ageing.

What a good morning regimen looks like

I advise all my patients — whether they are having injectable treatments or not — to use vitamin C consistently as part of a morning skincare routine. The order of application matters, and simplicity is usually more sustainable than complexity.

After cleansing, vitamin C serum should be applied to dry skin and left to absorb for a minute or two. This is then followed by moisturiser and, critically, a broad-spectrum SPF 30 or higher. This combination — antioxidant, hydration, sun protection — is the foundation of any regimen that is genuinely supporting the skin's long-term health.

In my practice, I use and recommend Obagi Professional-C serums, which are formulated with L-ascorbic acid at concentrations of 10%, 15% and 20%. They are medical-grade, properly formulated at the correct pH, and available in the packaging required to maintain stability. They are not the cheapest option on the market, but they are among the best-evidenced — and in skincare, you generally get what you pay for when it comes to active ingredients.

The key message is this: injectable treatments and home skincare are not alternatives to one another. They work best together. In-clinic treatments address the structural changes that topical skincare cannot reach. But topical skincare — done consistently and with effective ingredients at effective concentrations — maintains and extends those results in a way that nothing else can replicate.
