In clinic, one of the most common things I hear from women in their late 30s and 40s is some version of the same sentence: "My skin has changed and I don't know why." The products that used to work no longer do. Skin that was once oily might now feel dry and tight. Breakouts that disappeared in their 20s have returned — often in new places. There is a dullness that foundation cannot fix.

What is happening is not mysterious, but it is hormonal — and it is happening much earlier than most people realise. The changes that become obvious in the 40s often begin quietly in the mid-30s, well before any formal conversation about menopause takes place. Understanding the biology makes the changes feel less alarming and, more importantly, makes the solutions far clearer.

The hormonal shift: what is actually changing?

Oestrogen is one of the skin's most important regulatory hormones. It supports collagen production, maintains skin thickness, promotes hydration, regulates melanocyte activity, and helps sustain the integrity of the skin barrier. During perimenopause — the transitional phase that can begin years before the last period — oestrogen levels begin to fluctuate and gradually decline. By menopause, they have fallen significantly.

The consequences for the skin are both rapid and cumulative. Research indicates that women can lose up to 30% of their dermal collagen in the first five years following menopause — a rate of loss that far outpaces the gradual age-related decline that occurs throughout adulthood. Alongside this, levels of hyaluronic acid (the molecule responsible for the skin's ability to retain water) and elastin (which gives skin its recoil and bounce) also decline. These three losses — collagen, hyaluronic acid, and elastin — do not happen in isolation. Together they create a cascade of structural change throughout the skin.

The result is skin that is simultaneously thinner, drier, and less elastic. The compromised barrier allows moisture to escape more easily and irritants to penetrate more readily. Wrinkling accelerates — not just because of repeated expression, but because the underlying structural scaffolding has thinned. Sagging becomes more noticeable as volume and elasticity reduce. Wounds take longer to heal. The skin, in short, becomes more fragile.

Decreased hydration: more than just dry skin

The dryness associated with hormonal change is frequently dismissed as something that can be fixed with a richer moisturiser. In reality, it is a structural problem. Oestrogen plays a direct role in stimulating the production of hyaluronic acid within the dermis — the deep layer of skin that provides its structure and resilience. As oestrogen declines, so does the skin's intrinsic capacity to hold water. No amount of topical application can fully compensate for this, because the loss is occurring within the skin itself, not on its surface.

This is why many women find that their skin drinks in moisturiser but still feels tight an hour later. The issue is not the product — it is the skin's reduced ability to retain hydration at a deeper level.

Slower cell turnover: the dull, sallow complexion

Oestrogen also regulates the rate at which the skin renews itself. In younger skin, the cycle of cell turnover — old cells shedding, new cells rising to the surface — takes approximately 28 days. As oestrogen declines, this process slows significantly. Old, dead skin cells accumulate on the surface rather than shedding efficiently, creating a dull, sallow appearance that no amount of highlighter quite resolves.

This is also why products seem to stop working. Active ingredients simply cannot penetrate as effectively through a thickening layer of dead cells. Improving cell turnover — whether through retinoids, professional treatments, or both — is therefore one of the most impactful things you can do for the overall quality and radiance of the skin.

Pigmentation and sensitivity: the role of hormonal fluctuation

Hormonal fluctuations during perimenopause can trigger increased melanocyte activity — meaning the cells responsible for producing pigment become more reactive and unpredictable. The result is uneven pigmentation: patches of hyperpigmentation, a more mottled skin tone, or the appearance or worsening of melasma. This is compounded by years of cumulative UV exposure, which now begins to manifest more visibly as the skin's repair mechanisms slow.

At the same time, the compromised skin barrier means the skin becomes significantly more sensitive and reactive. Ingredients, products, and even environmental triggers that were previously well tolerated can suddenly cause redness, irritation, or stinging. Many women in this phase find themselves described as having "sensitive skin" for the first time in their lives — which is less a skin type than a reflection of a weakened barrier that needs support.

The androgen paradox: breakouts in your 40s

Perhaps the most confusing skin change for many women in this phase is the return of acne-like breakouts — often described as feeling like being a teenager again, but in a body that is simultaneously experiencing dryness and loss of volume. Understanding why this happens requires looking at the broader hormonal picture.

As oestrogen and progesterone decline sharply during perimenopause and menopause, the remaining testosterone — which women produce in smaller quantities throughout their lives — is left relatively unsuppressed. This shift creates what is known as relative hyperandrogenism: not an excess of testosterone in absolute terms, but an imbalance in which androgens (male hormones) now have a disproportionate influence on the body's biology, including the skin.

Androgens signal the sebaceous glands to produce sebum — the skin's natural oil. In a hormonally balanced state, this is regulated. Under relative hyperandrogenism, the sebaceous glands become overactive, producing excess thick oil that clogs the pores. Combined with the slower cell turnover described above — which means dead skin cells are not shedding efficiently — the result is blocked follicles, inflammation, and breakouts.

These breakouts are characteristically different from teenage acne. They tend to be deeper, more painful, and cystic in nature, concentrated along the jawline, chin, and neck rather than the forehead and nose. They can persist for weeks at a time and often do not respond to the gentle cleansers and light moisturisers that typically help younger acne-prone skin. The combination of excess oil production and impaired cell turnover is also why many women in this phase experience both breakouts and dryness simultaneously — a frustrating paradox that makes product selection genuinely difficult.

What you can do at home

A well-constructed home skincare routine will not reverse structural hormonal change, but it can meaningfully slow its progression, improve skin quality, and create a foundation that makes clinical treatments more effective. The following three categories are where I direct every client to begin.

SPF — daily, without exception

If there is one non-negotiable in a skincare routine, it is daily broad-spectrum SPF 30 or higher. UV radiation is the primary external driver of collagen degradation, pigmentation, and accelerated skin ageing — and at a time when the skin's internal repair systems are already under pressure from hormonal change, unprotected sun exposure compounds the damage significantly. SPF applied consistently over years is not a cosmetic nicety; it is a genuine investment in the long-term integrity of the skin. It also helps manage the hormonal pigmentation many women experience during perimenopause, which is markedly worsened by UV exposure.

Vitamin C — collagen support, protection and brightening

Vitamin C is one of the most important topical ingredients for hormonally ageing skin — and not only for the reasons most people associate with it. What is less widely understood is that Vitamin C is an essential co-factor in the body's collagen synthesis process. Without adequate Vitamin C, the skin cannot properly hydroxylate proline and lysine — two amino acids that are critical to forming stable, structurally sound collagen. In practical terms, this means that a well-formulated Vitamin C serum is not just a brightening product; it is actively supporting the skin's ability to produce and maintain new collagen at a time when that process is already under pressure from hormonal decline.

Beyond collagen synthesis, a stable Vitamin C serum applied in the morning works synergistically with SPF to neutralise free radicals generated by UV exposure — reducing the oxidative damage that breaks down existing collagen — and inhibits tyrosinase, the enzyme involved in melanin production, helping to gradually even out the hormonal pigmentation many women experience during perimenopause. For the dull, sallow complexion that accompanies slower cell turnover, it is one of the most effective brightening ingredients available. Look for formulations containing L-ascorbic acid at 10–20% concentration, or more stable derivatives such as ascorbyl glucoside if your skin is on the sensitive side.

Retinoids — the most clinically proven tool for ageing skin

Retinoids (Vitamin A derivatives) are the most evidence-backed skincare ingredient available for addressing the structural changes associated with hormonal ageing. They work by stimulating fibroblast activity and collagen production, accelerating cell turnover to address dullness and pigmentation, inhibiting the enzymes that break down existing collagen, and — for those experiencing hormonal breakouts — regulating sebum production and preventing the formation of blocked follicles.

For women in their 40s experiencing multiple skin concerns simultaneously, a well-tolerated retinoid used consistently is one of the highest-impact changes you can make to your routine. Start slowly — two to three nights per week — and build tolerance over several weeks. The initial purge and dryness that many people experience is temporary and is not a reason to stop. Home skincare supports and maintains; clinical treatments address structural change at a level that topical products simply cannot reach. The treatments I most commonly recommend for women navigating hormonal skin changes focus on four areas: deep hydration, barrier repair, collagen stimulation, and skin elasticity.

Skin boosters — restoring deep hydration

Skin booster treatments deliver hyaluronic acid directly into the dermis via microinjections, replenishing the very molecule that oestrogen decline has depleted. This is not surface hydration — it is structural rehydration at the level where the skin's resilience and plumpness originate.

Profhilo is one of the most established treatments in this category — a high-concentration, pure hyaluronic acid that disperses through the tissue to hydrate, firm, and stimulate the production of collagen and elastin. Teoxane Baby Glow — part of the RHA (Resilient Hyaluronic Acid) family — works similarly, delivering intense hydration and improving overall skin texture and luminosity. It is an excellent option for those whose primary concerns are dullness, fine surface lines, and dehydration, and works beautifully alongside a retinoid home routine.

Polynucleotides — rebuilding the skin barrier

Polynucleotide treatments (PDRN — polydeoxyribonucleotide) represent one of the most exciting developments in regenerative aesthetics. Derived from purified DNA fragments, polynucleotides work by stimulating fibroblast activity — the cells responsible for producing collagen, elastin, and hyaluronic acid. They have a particular affinity for improving skin quality at a cellular level: strengthening the skin barrier, improving hydration, increasing elasticity, and creating a healthier, more resilient skin environment.

For women in perimenopause and menopause, polynucleotides are especially relevant because they target the barrier compromise and loss of skin quality that drives so many of the visible changes — rather than simply addressing individual concerns such as lines or volume. The results build gradually over a course of treatments and are well suited to those whose skin has become fragile, reactive, or slow to recover.

Microneedling — stimulating renewal from within

Medical microneedling creates controlled micro-channels in the skin, triggering the body's natural wound healing response and stimulating the production of new collagen and elastin. It also significantly improves the penetration of active ingredients applied during the treatment. For women experiencing dullness, textural changes, enlarged pores, and the early signs of structural thinning, microneedling offers meaningful improvement in skin quality and overall radiance — and, crucially, it improves cell turnover in a way that addresses the underlying cause of the sallow complexion rather than simply treating its surface appearance.

Biostimulators — rebuilding collagen and elasticity

For women experiencing more significant structural change — noticeable laxity, volume loss, deepening lines — biostimulators such as Sculptra (poly-L-lactic acid) and Juvelook (a hybrid of hyaluronic acid and PDLLA microspheres) offer longer-term collagen regeneration and improved skin elasticity. Unlike dermal fillers, which replace lost volume directly, biostimulators work by stimulating the body's own collagen production over a period of weeks and months. The results are gradual, natural, and — importantly — address the underlying loss of structural integrity rather than compensating for it superficially.

These treatments are not about changing the face; they are about restoring the skin's capacity to support itself — which is exactly what hormonal change has diminished.


A note on treating the whole picture
